What the surveys are like

The WOREP survey is a two-part form. One part is filled out by the librarian and the other by the patron. Each pair of forms has the same unique number, allowing for comparison of the perceptions of the librarian and the patron when the forms are processed. The forms used for academic and public libraries differ only slightly in the questions asked. Samples for both are shown below.
